This session will explore this by discussing evidence-based practice, clinical review, complex cases, new research and other best practices.

1. Discuss the impact of systems on common acute and chronic conditions and on the ability to both prevent and manage them.
2. Adjust your practice habits to improve specific health outcomes among their patients.
3. Use your knowledge and skills to influence a positive change through advocacy at the individual patient level through engaging the interdisciplinary team at the practice level through engaging practice or hospital leadership, or at the local, state, or national level through engaging legislators.
