Annual conference theme: “Patient Centered. Community Focused”. The conference brings together healthcare leaders and professionals from Federally Qualified Health Centers (FQHCs), primary care medical facilities, health center controlled networks (HCCNs), and community partners across Georgia to explore innovative, evidence-based strategies that improve patient outcomes while strengthening community health. Through expert presentations, interactive discussions, and best-practice sharing, participants will examine approaches that advance whole-person care, population health, health equity, workforce development, quality improvement, financial sustainability, advocacy, and technology-enabled care. The conference is designed to foster interdisciplinary collaboration and equip attendees with practical tools to enhance access, quality, efficiency, and patient engagement in the evolving healthcare landscape.

Learning Objectives: 

At the conclusion of this conference, participants will be able to:

1. Implement patient-centered, evidence-based strategies that improve quality, safety, access, and health outcomes across diverse patient populations.
2. Integrate multidisciplinary approaches that address behavioral health, social drivers of health, and community partnerships to support whole-person care.
3. Apply population health and quality improvement principles to reduce disparities, improve preventive care, and enhance chronic disease management.
4. Leverage technology, health information systems, and data analytics to improve clinical performance, operational efficiency, and care coordination.
5. Develop collaborative leadership and organizational strategies that strengthen workforce engagement, financial sustainability, advocacy efforts, and organizational resilience while advancing the mission of community health centers.
