Fragmented care—when patients see multiple clinicians or receive care at multiple hospitals—results in poor outcomes for patients and the health system. This talk will review current evidence for the impact of fragmented care on older adults with Alzheimer’s Disease and provide a perspective on ways that clinicians and health systems can mitigate the negative effects of fragmented care.
